Heart, blood pressure, and nitrates

• Tadalafil and similar drugs dilate blood vessels
• They must never be combined with GTN (spray or tablets), isosorbide, riociguat, or recreational poppers (amyl nitrite)
• Recent heart attack, unstable angina, stroke, or being told sex is unsafe needs in-person cardiology or GP review first

Vision, penile anatomy, and infections

• Sudden vision loss in one eye or a history of NAION needs specialist advice before PDE5 inhibitors
• Peyronie's disease (curved penis) and past priapism (painful erection lasting hours) affect risk and dosing
• Burning urine, discharge, or new severe testicular pain may mean infection or other urgency, not just ED

• Adult men (18+) using the pathway for themselves
• No current emergency chest pain or breathlessness at rest
• Willingness to read the patient leaflet and seek 999 for priapism over four hours
• Accurate list of all medicines and recreational substances
